Ultra – Create/Deploy Form (Survey)

1. On the Course Content page, click the purple plus sign where you would like to create the new survey:

2. Select “Create” from the drop down menu:

3. Below “Assessment”, click “Form”:

4. Enter a new title for your form (survey) in the header:

5. To add content to the form (survey), click the purple plus sign:

6. Select from the options in the drop down menu:

7. To edit the form (survey) settings, click the gear icon next to “Survey Settings”:

8. In the Test Settings panel, review the preset options for due dates, student attempts, grades, etc. and according to your preferences:

9. Click “Save” when you have finished adjusting the settings:

10. By default, the test will be hidden from students. Click the drop down menu in the top right corner to adjust the visibility settings:
